MINUTE entry before the Honorable Lindsay C. Jenkins: Plaintiff's motion to proceed in forma pauperis 4 is granted because Plaintiff has represented to the Court that he is unemployed and has not received income from sources other than government assistance. He also represents that he has no cash in her savings account as well as some credit card debt, but no other assets. Paying the filing fee would prevent him from purchasing the necessities of life. Because his IFP application was granted, the Clerk is instructed to: (1) issue 1 summons for service of the complaint for the Defendant; and (2) send Plaintiff 1 blank USM-285 service forms. By March 3, 2025, Plaintiff shall submit a completed USM-285 forms to the Clerk's Office for the Defendant, which is required for service. The U.S. Marshal is appointed to serve the Defendant, but the U.S. Marshal will not attempt service on any Defendant unless and until the required form is received. The Court directs the Marshal to make all reasonable efforts to serve each Defendant. Plaintiff's failure to comply will result in the dismissal with prejudice of this lawsuit for want of prosecution.
